Instituted May 17th, 1941. A circular gilded zink medal, the obverse with a leftward facing depicting of King Zvonimir, circumscribed Dmitar Zvonimir and 1076, the reverse with a central checkered Croatian Coat of Arms within a laurel wreath, inscribed in Croatian and dated 1941, on an affixed Croatian crown for suspension from its period original tri fold ribbon featuring affixed oak twigs, unmarked, measuring 32. 3 mm
